1. Obtain a grant application.  Submit a completed COPIC Medical Foundation Grant Application or contact Kathy Brown.
   
2. Attach or include: 
   • Description of needs
   • Methods to be used in addressing the problem
   • Expected results
   • Budget
   • Name of organization as recognized by the Internal Revenue Service
   • Brief history and purpose
   • Copy of tax status 501(c)(3) letter (dated within past 10 years)
   • Total projects cost and amount of request to be considered
   • Sources of other funding
   • List of Board of Directors and Chief Administrator
   • Most recent financial statement
   • Most recent audit
   • Other information of value to the request

4. You will be expected to report on the progress of your program if a grant is awarded. This will be accomplished by returning a Grant Evaluation Report form, which will be sent to you for completion.
